Mace has argued that biological differences between men and women should be a key consideration in policies related to athletics, asserting that trans-identified males competing in women’s sports may create unfair advantages. She has advocated for legislation designed to preserve the integrity of women’s sports while expressing reluctance to finding equitable solutions that include trans-identified males in the spaces of women.  This position aligns with her broader pro-woman agenda, emphasizing the protection of opportunities and safety for women and girls.

On broader transgender issues, Mace has called for thoughtful dialogue and firm policymaking to protect women and girls. While she has supported certain initiatives aimed at protecting LGB rights, such as workplace protections, she has also urged, in her words, that the "LGB divorce the T", a sentiment felt by many members of the Lesbian, Gay and Bisexual community surrounding the "lumping" of Transexual males into the Gay and Lesbian community. Nancy Mace’s policy positions on transgender issues resonate with key principles of the gender-critical movement, a growing ideology that questions the implications of gender self-identification on women’s rights. This movement, which has gained momentum globally, argues that policies prioritizing gender identity over biological sex risk undermining hard-won protections for women. Advocates within the gender-critical movement often highlight issues such as the impact of gender identity policies on single-sex spaces, women’s sports, and legal definitions of sex. Nancy Mace has been coined a "SUPER TERF", and wears the label not only proudly, but boldly.
